How to Prepare Your Home for Home Health Care

Preparing your home before home health services begin ensures a smooth transition and optimal care environment. These practical steps will help create a safe, comfortable space for recovery.

Safety Modifications

Install grab bars in bathroom near toilet and in shower/tub
Remove tripping hazards like loose rugs and electrical cords
Improve lighting in hallways, stairs, and entryways
Consider a raised toilet seat and shower chair if needed
Ensure smoke detectors and carbon monoxide alarms are working

Accessibility Improvements

Clear wide pathways for walker or wheelchair access
Move frequently used items to easy-to-reach locations
Consider a hospital bed if getting in/out of regular bed is difficult
Ensure phone and emergency call button are within reach
Arrange furniture to accommodate medical equipment

Medication Management

Create a clean, well-lit medication storage area
Gather all prescriptions, over-the-counter meds, and supplements
Have an updated medication list ready for your nurse
Set up a pill organizer if you'll be managing some medications
Keep a list of pharmacy contact information handy

Documentation & Information

Compile medical records, test results, and hospital discharge papers
List all physicians with contact information
Have insurance cards and ID readily available
